How to structure clinical content so ChatGPT, Perplexity, Claude, and Google AI Overviews quote your clinic when patients ask about conditions you treat.
01 — Why AEO
Patients now ask, they don't search.
A patient with knee pain used to type “knee pain physio” into Google and scroll ten blue links. Today, an increasing share of that same patient types a full sentence into ChatGPT: “I've had knee pain for three weeks after running — should I see a physio or a doctor?” The AI answers directly, and quietly cites two or three sources.
If your clinic isn't one of those sources, you don't exist in that conversation. Answer Engine Optimization is the practice of engineering your content so answer engines choose you as the citation.
02 — Terrain
The four surfaces that matter.
- 01Google AI Overviews. The generative panel above the classic search results. Currently the largest answer surface for medical queries.
- 02ChatGPT web search. When ChatGPT enables browsing, it retrieves and cites specific URLs. Being one of those URLs is the goal.
- 03Perplexity. The most citation-heavy engine — every answer lists three to eight sources. Highest leverage for early-stage AEO work.
- 04Claude, Gemini, Copilot. Each maintains its own retrieval pipeline. Structure that works for one usually works for all.
Step 1
Write for extractability, not for rank.
Answer engines don't rank pages — they extract passages. The unit of AEO is the paragraph, not the page. Every important answer on your site should be lift-and-quotable on its own.
- One clear question per H2, phrased the way a patient would ask it.
- The direct answer in the first two sentences below the heading — before any preamble.
- 40–80 word answer blocks. Long enough to be complete, short enough to quote whole.
- State the entity by name in every answer (your clinic, your city, the condition) — pronouns break extraction.
Step 2
Schema is how machines confirm what humans see.
Structured data is the fastest way to tell a retrieval pipeline what a page is about. For a clinic, four schemas do most of the work:

Step 3
Be the source, not a re-tell.
Answer engines are trained to prefer primary sources over aggregators. A page written by a named physiotherapist about a condition she treats every week will outrank a generic health-content site that outsources articles to freelancers — even if the generic site has more traffic.
Write from clinical practice. Include what you actually see in consult: the mistake patients make in the first week, the question you get in every intake, the recovery timeline you tell people to expect. This is the signal answer engines cannot get from WebMD.
Step 4
Get cited off-site.
Retrieval pipelines lean heavily on off-site mentions to corroborate what a page claims. Two moves compound:
- A profile on the national health-council directory, with the same clinician bio and specialty language you use on your own site.
- A handful of Reddit answers from a clearly-identified clinician account. Answer engines index Reddit aggressively and cite it often for medical queries.
- Guest posts on adjacent local sites (a gym, a running club, a local news site) with a clean bio link back to your service page.
Step 5
Measure citations, not clicks.
AEO breaks the classic funnel. A patient can read your quoted passage inside ChatGPT, decide to book, and land on your site already ready to call — with no measurable referrer. Track:
- Monthly manual audit: run 20 patient-style questions through ChatGPT, Perplexity, and Google AI Overviews. Log whether your clinic is cited.
- First-touch attribution on the booking form: 'How did you first hear about us?' — 'ChatGPT / AI search' is now a real answer.
- Direct traffic and branded search: both usually climb before referral traffic does when AEO starts working.
